Terms of Service
1. Acceptance of Terms
By accessing rxgranted.com, creating an account, signing up for a service plan, or using any RxGranted service, you confirm that:
- You have read, understood, and agree to these Terms
- You are authorized to enter into this agreement on behalf of your medical practice or organization
- You are at least 18 years of age
- Your use of our services complies with all applicable laws and regulations
If you do not agree to these Terms, do not use our website or services.
2. Description of Services
RxGranted provides prior authorization documentation and administrative support services for independent medical practices. Services may include:
- Preparation of clinically justified prior authorization letters
- Medical necessity documentation and supporting clinical write-ups
- Appeal letters for denied prior authorization requests
- Priority or urgent processing, where applicable
- Direct PA submission and payer follow-up (Full Service plan only)
RxGranted offers service tiers including Core, Core+, and Full Service. Specific plan features, monthly case limits, included portal users, overage terms, add-on seats, and pricing are set forth in the applicable Service Agreement or enrollment confirmation provided to Client at onboarding.
Core and Core+ clients submit completed PA letters to the payer unless otherwise agreed in writing. Full Service clients may authorize RxGranted to submit PA requests directly to payers and perform payer follow-up on Client's behalf.
RxGranted provides documentation and administrative support services only. We do not provide medical advice, clinical care, or treatment recommendations. We do not prescribe, dispense, or administer medications. We do not act as a healthcare provider to any patient. All clinical decisions remain solely with the treating licensed provider.
RxGranted does not guarantee that any prior authorization request will be approved. Coverage decisions are made exclusively by the patient's insurance company.
- RxGranted relies on the accuracy and completeness of information provided by Client and does not independently validate underlying clinical records.
- RxGranted makes no warranty regarding payer approval, payer timing, appeal success, or clinical outcome.
- All payer decisions are made solely by the applicable insurer or reviewing body and are outside the scope of RxGranted's services.
3. Account Responsibilities
When you create an account or use our services, you agree to:
- Accurate information: Provide truthful, accurate, and complete information about your practice, NPI, specialty, and patient cases. Inaccurate information may delay or invalidate PA services.
- Login security: Maintain the confidentiality of your portal credentials. Do not share your password. You are responsible for all activity that occurs under your account.
- Unauthorized access: Notify us immediately at [email protected] if you discover or suspect any unauthorized access to your account.
- Authorized representative: Only authorized personnel of the medical practice may access the portal and submit PA cases.
- HIPAA obligations: Execute a Business Associate Agreement (BAA) before submitting any Protected Health Information, and ensure your practice maintains HIPAA compliance in the transmission of patient data.
4. Payment Terms
Billing
Pricing is not published on the RxGranted website and is quoted individually based on the Client's selected plan, service needs, expected PA volume, portal user requirements, and any agreed add-ons.
The applicable monthly subscription rate, included PA volume, overage rate, add-on seat pricing, annual billing terms, and any other billing terms are set forth in the Client's Service Agreement or enrollment confirmation.
RxGranted offers Core, Core+, and Full Service plans. Plan limits generally include:
- Core: up to 6 PA letters per month and 2 included portal users
- Core+: up to 8 PA letters per month and 3 included portal users
- Full Service: up to 14 PA cases per month and 5 included portal users
Additional portal users may be available at the per-seat monthly rate specified in the Client's Service Agreement or enrollment confirmation.
Overage PA cases beyond plan limits are billed at the per-case rate specified in the Client's Service Agreement or enrollment confirmation.
Annual billing may be available at a discounted rate as specified in the Client's Service Agreement or enrollment confirmation.
Proof of Concept Period
- Each plan includes a Proof of Concept period lasting thirty (30) days or five (5) PA cases, whichever occurs first.
- Client is required to keep a valid credit card on file at enrollment, but Client is not charged the monthly subscription fee during the Proof of Concept period.
- The Proof of Concept period is a professional evaluation period attached to the Client's selected plan and includes the features of that selected plan, subject to applicable operational limitations and Client's timely submission of complete case information.
- After the Proof of Concept period ends, billing begins at the negotiated rate set forth in the Client's Service Agreement or enrollment confirmation, unless the Client cancels before the Proof of Concept period ends.
Price Lock
- Client's quoted subscription rate is guaranteed for twelve (12) months from the Effective Date of the applicable Service Agreement or enrollment confirmation.
- After the initial twelve (12) month period, RxGranted may adjust pricing upon at least sixty (60) days' written notice to Client.
Refund Policy
- No refunds are issued for completed PA letter work, submitted PA cases, appeal work, or other completed services
- No refunds are issued for partial months
- If Client cancels after billing has begun, access continues through the end of the paid billing period unless Client requests earlier termination or RxGranted terminates access for cause

Cancellation
- Client may cancel a subscription at any time by providing written notice to [email protected]. Cancellation is effective at the end of the then-current billing cycle, unless Client requests immediate termination.
- Upon cancellation, RxGranted may, in its discretion, complete in-progress PA cases accepted before cancellation, provided Client remains in good standing and all required information has been received
- Upon termination, PHI will be returned or securely destroyed in accordance with the applicable Business Associate Agreement
- RxGranted reserves the right to suspend services for accounts more than thirty (30) days past due
5. Intellectual Property
All content on rxgranted.com — including but not limited to text, design, graphics, logos, PA letter templates, clinical documentation frameworks, and software — is the exclusive property of ASKARX LLC and is protected by applicable copyright, trademark, and intellectual property laws.
- You may not copy, reproduce, modify, distribute, or create derivative works from our content without express written permission
- PA letters and documentation we prepare for your patients remain the property of ASKARX LLC until delivered; upon delivery, you receive a license to use them solely for the described patient's insurance submission
- "RxGranted" and the RxGranted shield logo are trademarks of ASKARX LLC
6. Limitation of Liability
- RxGranted is not responsible for payer coverage decisions. Approval of any prior authorization request is ultimately determined by the patient's insurance company.
- RxGranted does not guarantee that any PA request will be approved.
- RxGranted shall not be liable for delays caused by incomplete or inaccurate information provided by Client.
- Except in cases of gross negligence, willful misconduct, or breach of Protected Health Information obligations, RxGranted's aggregate liability shall not exceed the total fees paid by Client in the twelve (12) months preceding the event giving rise to the claim.
- In no event shall RxGranted be liable for indirect, incidental, special, consequential, or punitive damages, including loss of revenue or profits, even if advised of the possibility of such damages.
- Client acknowledges that RxGranted's role is limited to documentation and administrative support, and that all clinical decisions remain solely with the treating provider.
7. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of New Jersey, without regard to its conflict of law provisions, to the extent not preempted by applicable federal law (including HIPAA).
Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the state and federal courts located in Mercer County, New Jersey, and you consent to personal jurisdiction in those courts.
8. Miscellaneous
- Entire Agreement: These Terms, together with our Service Agreement, HIPAA BAA, and Privacy Policy, constitute the entire agreement between you and RxGranted regarding our services
- Severability: If any provision of these Terms is found unenforceable, the remaining provisions continue in full force
- Waiver: Failure to enforce any provision does not constitute a waiver of that provision
- Updates: We may update these Terms periodically. Material changes will be communicated by email. Continued use of services after the effective date constitutes acceptance.

Electronic Signatures
These Terms may be accepted electronically. Clicking to accept, creating an account, or using the services constitutes electronic agreement and shall have the same legal force and effect as a written signature.
